Requirements

- Ability to navigate multiple software systems while completing documentation during live interactions.
- Comfortable learning new technologies and working with digital healthcare platforms.
- Proficiency with Google Workspace or similar productivity tools.
- Highly organized with the ability to manage complex information flows.
- Ability to work independently in a fully remote environment.
Responsibilities
- Conduct outreach to individuals expressing interest in healthcare services and explain available programs and resources.
- Perform initial consultations and gather patient or caregiver information to support care coordination.
- Coordinate cognitive health screening initiatives and assist with enrollment campaigns.
- Collect medical history while maintaining accuracy and confidentiality.
- Schedule and coordinate appointments between patients, caregivers, clinical teams, and external partners.
- Provide guidance and technical support to patients navigating digital healthcare platforms.
- Maintain accurate documentation across multiple systems while managing real-time information during calls.
