Requirements
- 3+ years experience in case management, care coordination, or digital health coaching.
- 1–2 years of experience in a lead or supervisory capacity.
- Working knowledge of HEDIS measures and ability to translate specifications into operational protocols.
- Experience managing or mentoring front-line care teams in a metrics-driven environment.
- Strong analytical skills, specifically with cohort dashboards and identifying trends.
- Excellent written and verbal communication skills for scripts and partner presentations.
- Experience in digital health or health tech environments preferred.
- Clinical or community health background (RN, LPN, LSW, or CHW) preferred.
- Experience with care management platforms, CRM tools, or health plan quality reporting systems preferred.
Responsibilities
- Oversee day-to-day operations of the Member Care Advocate team.
- Develop analytical and programmatic infrastructure to turn engagement data into targeted care interventions.
- Carry a personal caseload of complex member cases to model performance standards.
- Define and maintain data standards for member measure status and outreach history.
- Translate PDC-clock math and HEDIS measure timelines into daily team prioritization queues.
- Hire, onboard, develop, and coach a team of advocates in a metrics-driven environment.
- Collaborate with Product and Engineering to optimize the Member Care Queue interface.
- Serve as the primary point of contact for cross-functional teams including Client Success, Quality, and Operations.
