Requirements
- At least 2 years of experience managing large individual contributor teams, ideally in healthcare or community care.
- 2+ years of experience with community engagement, patient navigation, or social work.
- Demonstrated experience working with individuals with complex chronic needs, such as homelessness, serious mental illness, or substance use disorder.
- Bilingual proficiency in English and Spanish.
- Strong cultural fluency and knowledge of community resources.
- Proficiency with CRM databases, Excel, Word, and video conferencing tools.
- Must maintain a quiet, HIPAA-compliant at-home work environment.
- Reliable internet and cell phone access.
Responsibilities
- Lead and develop a team of Lead Care Managers through 1:1 coaching and management.
- Conduct bi-weekly audits of care management work to ensure quality documentation and care plans.
- Monitor performance metrics via dashboards and manage escalations or troubleshooting.
- Develop and implement training programs and maintain Standard Operating Procedures.
- Act as a liaison between care teams, partner clinics, and community support organizations.
- Drive process improvements to enhance care management and coordination efforts.
