Requirements
- Active and unencumbered compact RN license.
- BSN or MSN degree from an accredited school of nursing.
- Minimum 4 years of nursing experience in hospitals, FQHCs, ambulatory care, or health payor systems.
- Experience in triage, intake, case management, value-based care, or population health preferred.
- Skilled in motivational interviewing and driving behavior change.
- Proficiency with digital tools, internet navigation, and multi-tasking across electronic documentation systems.
- Demonstrated ability to manage large caseloads in a fast-paced environment.
- Experience in high-growth or rapidly changing environments.
- Strong professional telephone and video communication skills.
- Ability to work Sunday-Thursday, 9am-5pm schedule.
Responsibilities
- Review member medical history, lab results, and medications.
- Follow intake protocols, standard processes, and clinical assessment decision trees.
- Support members through the enrollment process in collaboration with a cross-functional intake team.
- Provide initial clinician review and escalate to providers when necessary.
- Collaborate with nurses, health coaches, providers, and operations teams to drive a seamless member experience.
- Utilize nursing processes to triage member alerts and inquiries.
- Maintain clinical notes, records, and member communications accurately.
- Contribute to the improvement of clinical care standards and participate in performance improvement activities.
