Requirements
- Prior experience in an admissions, intake, patient coordination, or case management role.
- Proven experience working with U.S.-based organizations.
- Strong ability to manage high call volumes while maintaining accuracy and professionalism.
- Experience working with EHR platforms, CRM systems, or referral tracking tools.
- Prior remote work experience with proficiency in tools like Slack, Zoom, and Google Workspace.
- Ability to respond to unpredictable, high-pressure situations with calm and clarity.
- Excellent verbal communication skills with a direct, professional style.
- Background in recovery, addiction treatment, or behavioral health (preferred).
- Familiarity with clinical language, treatment levels of care, and insurance verification (preferred).
- Demonstrated ability to work independently and take ownership of outcomes.
Responsibilities
- Serve as the primary point of contact for all inbound calls from hospitals, referral partners, families, and individuals.
- Conduct thorough eligibility screenings based on established program criteria.
- Gather and document medical history, functional status, insurance details, and discharge context.
- Maintain real-time visibility into all referral statuses using EHR systems and tracking tools.
- Coordinate next steps including assessments and internal reviews for incoming referrals.
- Provide clear, honest direction and redirect callers to external resources when needs fall outside program scope.
- Escalate complex cases and identify process gaps to improve operational efficiency.
