Requirements
- New Jersey LCSW, LMFT, or LPCC
- 2+ years of health care social work experience
- Experience in hospice, palliative care, oncology or home health
- Strong understanding of cultural competency with medicaid population
- Excellent oral and written communication skills
- Proficiency in EMR systems, Salesforce, and Athena
- Working knowledge of MS Office and G-Suite
Responsibilities
- Conduct comprehensive psychosocial assessments through video and telephone
- Provide grief and bereavement support and counseling
- Screen for mental health concerns and safety risks
- Participate in weekly IDT meetings
- Perform medical social work case management
- Assist families in navigating healthcare systems and community resources
- Facilitate advance care planning and POLST completion
- Maintain patient documentation within 48 hours
