Requirements
- 3 years of experience in health care coordination, community health work, or similar field.
- Experience with a team-based approach to care.
- Excellent working knowledge of community resources.
- Ability to prioritize and task-switch.
- Self-directed and motivated.
- Strong communication and teaching skills.
- Intermediate MS Office skills including Outlook and Teams.
- Minimum internet download speed of 25 Mbps and upload speed of 10 Mbps.
- Dedicated workspace to protect PHI/HIPAA information.
Responsibilities
- Perform telephonic outreach to enroll and engage patients.
- Develop and implement care plans in collaboration with Nurse Care Managers.
- Facilitate seamless transitions of care across care settings and dialysis centers.
- Conduct assessments to identify social determinants of health.
- Connect members to community resources such as housing and food security programs.
- Provide patient advocacy by identifying and addressing barriers to care.
- Maintain documentation of patient interactions and care plans.
- Educate and empower patients to manage their conditions.
