- Serve as a trusted partner to nephrology and primary or specialty practices on HEDIS and Stars expectations, documentation standards, and value-based care priorities.
- Share and interpret quality reports, care gap lists, and dashboards to translate data into actionable improvement steps.
- Educate provider practices on documentation accuracy, coding completeness, and best practice workflows.
- Facilitate recurring meetings with providers and practice staff to resolve workflow barriers and develop action plans.
- Manage an assigned panel of patients, monitoring and prioritizing HEDIS and Stars care gaps across preventive care and chronic disease management.
- Conduct proactive patient outreach to educate, motivate, and support patients in completing screenings, labs, and appointments.
- Identify and address patient-level barriers such as transportation or care plan confusion.
- Collaborate cross-functionally with Quality, Clinical Operations, Coding, and Pharmacy teams.
