Requirements
- Active and unrestricted RN License in California.
- 3–5 years of clinical nursing experience (PACE, geriatrics, or home health preferred).
- Experience with ILOC determination processes and DHCS PACE submission requirements.
- Strong Epic clinical documentation and workflow management experience.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
- Ability to travel to conduct in-person ERN quality meetings as needed.
- Current CPR/BLS certification or ability to obtain within 30 days.
- Medical clearance for communicable diseases (including TB test) and up-to-date immunizations.
Responsibilities
- Own daily review of the RN tracker, Salesforce enrollment pipeline, and Epic work queues.
- Monitor completed, upcoming, canceled, and rescheduled Enrollment RN home visits.
- Identify stalled cases, missing documentation, and scheduling gaps.
- Serve as the central operational reviewer for all ILOC submissions.
- Triage Level 2 case flags and confirm documentation completeness.
- Own end-to-end Health & Safety Committee operations for Southern California.
- Enforce Pre-Enrollment SOPs and deliver training to Enrollment RNs.
- Coordinate Enrollment RN schedules and home visit assignments.
- Maintain enrollment operations during leadership absences and participate in PACE audits.
