Patient Enrollment Specialist

High school diploma or equivalent required; associate or bachelor's degree in healthcare, communications, social work, or a related field preferred. 1–3 years of experience in healthcare outreach, patient education, admissions, or other patient-focused position. Prior work in home health, hospice, managed care, or chronic condition programs preferred. Strong verbal and written communication skills with a persuasive, empathetic approach. Ability to clearly communicate medical and logistical information in patient-friendly language. Confidence in handling objections and navigating sensitive conversations. Comfortable working with EMR systems, Athena preferred. Familiarity with motivational interviewing or patient-centered communication techniques. Passion for helping vulnerable or underserved populations access care. Bilingual abilities (especially Spanish) are a plus.

Initiate outreach to patients and caregivers to explain home-based care benefits. Build rapport with hesitant patients and address objections. Use motivational interviewing and empathy-driven communication. Review and respond to incoming referrals, conducting proactive outreach. Guide patients through the enrollment process (eligibility, paperwork, consent). Coordinate with care teams, discharge planners, physicians, and social workers. Reconnect with undecided patients to reassess readiness. Maintain accurate records of outreach efforts and patient status in EMR systems.
