Responsibilities:
Provide medication management including reconciliation, dose optimization, and adherence counseling. Collaborate with interdisciplinary care teams to develop individualized care plans. Monitor medication regimens for clinical criteria, goals, adverse effects, and interactions. Identify opportunities for drug therapy optimization and cost savings. Participate in quality improvement initiatives and outcome studies. Optimize resources and workflows for prior authorizations and appeals. Collaborate with healthcare providers to ensure seamless medication management. Perform clinical assessments and educational counseling for targeted patients. Ensure clinical compliance with FDA guidance and clinical guidelines. Assist with workflow optimization initiatives. Advocate for patient safety and medication literacy. Maintain confidentiality of sensitive information. Update job knowledge through continuous learning and professional development.
