Deliver CCM and APCM services, including developing and maintaining comprehensive, patient-centered care plans.
Conduct regular patient check-ins, perform medication reconciliation, and coordinate community or social-service referrals.
Build motivational relationships with patients to address health goals and social determinants of health.
Document all encounters accurately, including service times and diagnostic codes to ensure billing compliance.
Maintain audit-ready patient records in accordance with HIPAA and program-compliance standards.
Collaborate with supervising practitioners and clinic teams to escalate urgent clinical issues.
