Requirements
- 8+ years of healthcare operations leadership experience.
- Experience leading Community Health Workers, care coordinators, patient navigators, outreach teams, or other community-based healthcare workforces.
- Experience building and managing distributed field teams.
- Strong understanding of Medicaid populations and social determinants of health.
- Demonstrated success developing frontline leaders and improving operational performance.
- Excellent communication, coaching, and change management skills.
- Experience with ECM, Community Supports, CalAIM, or other Medicaid care management programs preferred.
- Experience working with culturally diverse and historically underserved populations preferred.
- Multi-state operational experience preferred.
- Experience scaling community-based healthcare teams during periods of growth preferred.
- Bilingual Spanish/English strongly preferred.
Responsibilities
- Lead all Community Health Worker operations across markets, including workforce planning, deployment, productivity, and performance management.
- Establish consistent standards for home visits, community-based interventions, member engagement, outreach, and field documentation.
- Build management systems that support coaching, accountability, quality, and professional development across the CHW organization.
- Partner closely with Nurse Practitioners, RN Care Managers, Behavioral Health Case Managers, and Care Coordinators to ensure seamless integration within care teams.
- Monitor operational performance and identify opportunities to improve member engagement, retention, and outcomes.
- Support expansion into new markets while maintaining consistency of service delivery and operational excellence.
- Serve as the primary leader responsible for the effectiveness and scalability of the CHW function.
