Requirements
- Current RN license required (BSN preferred).
- Minimum of 5 years of experience as an RN.
- Minimum of 2+ years of experience as an RN in palliative medicine.
- Minimum of 2+ years of experience in care plan creation, proactive discharge planning, and utilization.
- Current CPR or BLS certification.
- Proficiency in EMR systems (Cerner) and Microsoft Office (Teams, Excel).
- Experience with virtual communication platforms.
- Home office with internet connectivity (minimum 1MB upload/download speed).
- Renal nursing experience (preferred).
- Compact nursing license (highly preferred).
Responsibilities
- Lead complex Goals of Care (GOC) and advance care planning discussions.
- Navigate patients to comfort resources and assist with healthcare decision-making.
- Coordinate care between dialysis centers, nephrologists, primary care, and external agencies.
- Assess symptom burden and implement nursing interventions to improve quality of life.
- Educate patients on advanced directives, curative treatments, and hospice care options.
- Manage a caseload of high-complexity patients and facilitate transitions to external services.
