- 5+ years of clinical practice experience
Requirements
- Board-certified Family Nurse Practitioner or Physician Assistant.
- 5+ years of clinical practice experience in a Primary Care setting.
- Licensed in NY.
- Minimum 1 year of experience using Epic EMR.
- Currently enrolled or eligible for enrollment as a Medicare provider.
- Experience in behavioral health and treating both acute and chronic conditions.
- Telemedicine experience preferred.
- Tech-savvy, proactive, organized, and detail-oriented.
- Clean background and medical malpractice history.
- Commitment of 40 hours per week, with 36 clinical hours.
Responsibilities
- Provide video-based, comprehensive clinical care covering Acute Care, pediatrics, and mental health.
- Support physicians with Epic inbox management including labs, imaging, and patient questions.
- Educate patients on appropriate treatments and care plans for their health needs.
- Prescribe medication and refills as appropriate.
- Collaborate with Care Team Members to drive high-quality care and patient outcomes.
