Requirements
- Associate's Degree in Health Care, Business, or related field.
- Minimum of 1 year of customer service experience, preferably in the healthcare industry.
- Experience entering data, navigating, and retrieving information from an electronic medical record (EMR).
- Familiarity with platforms such as Epic and Careport Care Management.
- Ability to manage multiple priorities in a team-based environment.
- Excellent verbal and written communication skills.
- Strong customer service orientation.
- Ability to adapt to change and adjust to shifting work assignments.
- Knowledge of managed care, Medicare, Medicaid, or insurance authorization processes is preferred.
- Experience in home care intake or placements is highly desirable.
Responsibilities
- Process preadmission and inpatient referrals in Epic and coordinate services for international patients.
- Coordinate out-of-area home care referrals via Careport Care Management to ensure timely start of care.
- Verify and submit clinical documentation to designated home care agencies.
- Identify home care providers that align with age-specific and population-specific patient needs.
- Manage face-to-face encounter forms and arrange home labs as appropriate.
- Respond to electronic platforms, emails, and phone inquiries to ensure referral requests are documented.
- Resolve non-clinical issues and complaints in a timely manner.
- Oversee the home care phone line and provide responsive intake support.
