Requirements

  • Associate's Degree in Health Care, Business, or related field.
  • Minimum of 1 year of customer service experience, preferably in the healthcare industry.
  • Experience entering data, navigating, and retrieving information from an electronic medical record (EMR).
  • Familiarity with platforms such as Epic and Careport Care Management.
  • Ability to manage multiple priorities in a team-based environment.
  • Excellent verbal and written communication skills.
  • Strong customer service orientation.
  • Ability to adapt to change and adjust to shifting work assignments.
  • Knowledge of managed care, Medicare, Medicaid, or insurance authorization processes is preferred.
  • Experience in home care intake or placements is highly desirable.

Responsibilities

  • Process preadmission and inpatient referrals in Epic and coordinate services for international patients.
  • Coordinate out-of-area home care referrals via Careport Care Management to ensure timely start of care.
  • Verify and submit clinical documentation to designated home care agencies.
  • Identify home care providers that align with age-specific and population-specific patient needs.
  • Manage face-to-face encounter forms and arrange home labs as appropriate.
  • Respond to electronic platforms, emails, and phone inquiries to ensure referral requests are documented.
  • Resolve non-clinical issues and complaints in a timely manner.
  • Oversee the home care phone line and provide responsive intake support.
