Requirements
- Master’s degree in Social Work (MSW) from a CSWE-accredited program.
- Current, unrestricted master’s-level social work license (e.g., LCSW/LISW).
- Minimum 3 years’ clinical social work experience in a healthcare setting.
- Proven competence in risk assessment and crisis management.
- Strong telehealth and phone clinical skills.
- Excellent clinical judgment and ability to make time-sensitive decisions while working remotely.
- Willingness to obtain additional state licenses as needed for cross-state practice.
- Proficiency with video visits, Google Suite, Slack, and electronic health records.
- Ability to work a schedule of four 10-hour workdays, including Monday, Friday, and Saturday.
Responsibilities
- Serve as the primary intake clinician for escalated behavioral health and advanced social needs consults.
- Complete time-sensitive psychosocial and safety/risk assessments via phone, video, or ThymeBox.
- Deliver brief, targeted interventions for stabilization and safety planning including suicide risk management.
- Conduct comprehensive psychosocial assessments to address emotional concerns and social barriers.
- Utilize clinical judgment to determine escalation pathways for behavioral health crises and social safety events.
- Coordinate with care teams and community organizations to address barriers like housing and financial needs.
- Maintain high-quality documentation of triage assessments, safety plans, and referrals.
- Provide clinical consultation and decision support to Care Team members.
