Requirements
- Board certified in medical oncology.
- At least 3 years of clinical experience caring for oncology patients after fellowship training.
- Valid, unrestricted medical license.
- Willingness to obtain additional state medical licenses as needed.
- Experience with transitional care management programs, care transition protocols, or reducing readmissions preferred.
- Proficiency with video chatting, Google Suite, and electronic health records (EHR).
- Strong listening and communication skills.
- Ability to work effectively in a fast-paced, ambiguous start-up environment.
- Demonstrated ability to prioritize and address urgent member needs.
Responsibilities
- Drive the evolution and clinical vision of the transitional case management program.
- Design clinical protocols, escalation pathways, and performance indicators for transitional care.
- Partner with product and technology teams to integrate evidence-based care tools.
- Provide clinical expertise and guidance to the frontline care team during case conferences and complex discharges.
- Facilitate training, education, and ongoing clinical updates for the care team.
- Conduct virtual visits with members as needed.
- Lead strategic initiatives to reduce avoidable readmissions and emergency utilization.
- Represent the organization in external meetings with health systems, payers, and partners.
