Responsibilities
- Engaging eligible patients through telephonic, written, and digital outreach methods
- Leveraging available data from EMR to learn more about potential patients and inform outreach processes
- Completing the intake and consent process for eligible patients, and transitioning them to a Care Coordinator upon full enrollment
- Engaging and fostering trust quickly with patients, acting as the face of Synapticure
- Connecting authentically with patients and their families in a non-judgmental, respectful, and empathic manner
- Working daily within EMR to collect data, organize information, track tasks, and communicate with staff and patients
- Providing feedback to the product development team
