Requirements
- Graduate of an accredited nursing program (ADN or BSN).
- Active, unrestricted RN nursing license in New York.
- 2+ years of experience in a health plan, health care organization, or social services.
- Knowledge of Medicare, Medicaid, and DSNP regulatory requirements.
- Experience delivering care management services in a telephonic/virtual environment.
- Understanding of elder care dynamics and underserved populations.
- Proficiency with Microsoft Office applications.
- Experience with care coordination, transitions of care, and motivational interviewing.
- Strong organizational, communication, and analytical skills.
- Ability to work in a fully remote environment.
Responsibilities
- Orient new members to the Belong Health DSNP program and educate on services.
- Conduct health risk assessments, reassessments, and clinical evaluations.
- Develop and implement an individual care plan (ICP).
- Identify members requiring intensive care management and coordinate services.
- Lead interdisciplinary care team meetings.
- Support transitions of care, including post-discharge outreach and medication reconciliation.
- Collaborate with providers, social workers, and community service organizations.
- Document care management activities in accordance with procedures.
- Utilize care management technology platforms to prioritize interventions.
