- 5+ years work experience in a healthcare role (e.g., Social Work, Patient Advocacy)
- Fluency in French and English
- Strong ethical standards and respect for client confidentiality
- Knowledge of healthcare sector navigation
- Experience developing and facilitating care plans
- Experience in high-touch customer journey support
- Proficiency with macOS, GSuite, Slack, and 1Password
Responsibilities
- Advise and assist families with healthcare-related tasks.
- Develop a strong understanding of the unique caregiving needs of Canadian families.
- Coordinate healthcare activities including providers, appointments, insurance, and billing.
- Create customized care plans for members.
- Collaborate with team members to deliver seamless service.
- Update Wellthy guides and community resources.
- Contribute to product and service improvement feedback.
- Engage in continuous professional development.
- Manage caseload hours to ensure consistent service levels.
