Requirements
- Masters degree in social work, counseling, or related field.
- Licensed to practice in the state of California as an independent clinician (LCSW, LPCC, or equivalent).
- Willingness to acquire additional state licenses.
- Minimum of 5+ years of clinical experience.
- Minimum of 3+ years of lead or supervisory experience.
- Proficiency in evidence-based treatment approaches (Problem-Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, Behavioral Activation).
- Technical proficiency in EHRs and ability to learn new technology.
- Knowledge of behavioral health assessment tools (PHQ9, GAD7, CSSRS).
- Experience with Collaborative Care Model (CoCM).
- Ability to provide clinical care to pediatric and adult populations.
Responsibilities
- Oversee up to 15 Collaborative Care Clinicians including performance management and clinical supervision.
- Support pod and partner workflows.
- Manage employee schedules, time off, and attendance.
- Serve as a resource for complex patient situations and escalated employee relations.
- Manage a small clinical caseload of up to 15 patients.
- Screen and assess patients using tools like PHQ9, GAD7, and Columbia Suicide Risk Assessment.
- Monitor patient symptoms and coordinate care with primary care providers.
- Document patient progress in Salesforce and electronic health records.
