Requirements
- Active RN license in the State of Florida (compact/multistate accepted)
- 3+ years of care coordination, case management, or clinical operations experience
- 1+ year of team lead or supervisory experience in a healthcare setting
- Experience with electronic health records (EHR/EMR) systems
- Proficiency with Microsoft Teams, Outlook, and standard office applications
- Strong knowledge of Medicare/Medicaid billing, CCM/RPM/BHI services, and home health order workflows
- Excellent organizational skills
Responsibilities
- Supervise and mentor a team of Care Coordinators, including onboarding and training.
- Serve as first point of escalation for team questions on referrals, authorizations, and clinical workflows.
- Coordinate daily huddles and team coverage schedules.
- Process and route incoming referrals, lab orders, and home health orders.
- Manage relationships with external home health agencies and labs.
- Coordinate DME orders and medication refills.
- Maintain accurate patient records in EHR (OA).
- Triage patient calls and manage provider communication channels.
- Support billing workflows for CCM, RPM, and BHI.
