Requirements
- Active, unencumbered Registered Nurse (RN) license.
- Current BLS certification.
- Recent experience in a clinical or chronic care coordination setting.
- High proficiency in Electronic Health Record (EHR) systems and Google Suite.
- Ability to work in a private, HIPAA-compliant home office.
- High-speed internet access.
- Dual-monitor setup.
Responsibilities
- Serve as primary point of contact for assigned patient panel.
- Conduct patient outreach and build rapport over the phone.
- Accurately capture health data.
- Guide patients toward self-management goals.
- Maintain high-quality clinical documentation.
- Monitor patient health status for changes requiring intervention.
- Collaborate with providers to improve patient adherence to care plans.
