- Minimum High School Diploma.
- Knowledge of medical terminology.
- High level of organizational skills.
- Proven follow-up ability.
- Excellent written and verbal communication skills.
- Proficiency in Electronic Health Record Systems.
- Proficiency in Microsoft Outlook and Microsoft Excel.
- Ability to use internet and cloud-based applications.
- Ability to communicate effectively with facility staff and medical team members.
- Dedicated quiet space with reliable internet access.
Responsibilities
- Perform remote monitoring and principal care management for patients under medical team supervision.
- Track and monitor patient data communication across medical team members.
- Engage with patients and family members to report clinical summaries.
- Assist clinicians with gathering medical information using electronic health record systems.
- Report monthly medical concerns such as falls, medication, behavioral issues, and hospitalizations.
- Complete PCM assignments for approximately 15 patients per day.
- Escalate issues to the ancillary program manager or Chief Clinical Officer.
- Provide training and support to newer employees via shadowing.
