Requirements
- Active, unencumbered Registered Nurse (RN) license.
- Must reside within the MST or PST time zone.
- Current BLS certification.
- Recent experience in a clinical or chronic care coordination setting.
- High proficiency in Electronic Health Record (EHR) systems.
- High proficiency in Google Suite.
- Private, HIPAA-compliant home office.
- High-speed internet and dual-monitor setup.
Responsibilities
- Serve as the primary point of contact for an assigned panel of patients.
- Build rapport over the phone and guide patients toward self-management goals.
- Accurately capture patient health data.
- Maintain high-quality documentation in EHR systems.
- Meet daily outreach goals.
- Proactively identify changes in patient health status requiring provider intervention.
- Facilitate patient and family education regarding chronic condition management.
