- Recent experience in a clinical or chronic care coordination setting
- High proficiency in Electronic Health Record (EHR) systems

- Dual-monitor setup
Responsibilities
- Serve as the primary point of contact for an assigned panel of patients.
- Build rapport over the phone and capture health data.
- Guide patients toward self-management goals.
- Manage patient outreach independently after training.
- Maintain high-quality documentation.
- Meet daily outreach goals.
- Identify health status changes requiring provider intervention.
