Requirements

  • Active, unrestricted RN license
  • Ability to obtain California licensure within 6 months of hire
  • Willingness to obtain additional state licensure as needed
  • 2 years of people management experience (4 years preferred)
  • 2-3 years of care management experience
  • 1-2 years of post-acute experience
  • Proven, results-driven leader with experience in change management in rapid-growth environments
  • Strong organizational and independent working skills
  • Proficient in Microsoft applications
  • Proficient in Google applications
  • Excellent communication and motivational skills
  • Conflict resolution techniques
  • Working knowledge of lean Six Sigma methodology is preferred
  • Willing and able to work Monday through Friday 8:30am-5:00pm Pacific Time, with flexibility for potential evenings and weekends

Responsibilities

  • Supervise a team of approximately 10-15 direct reports under the ECM team to ensure high-quality enrollments and care management services
  • Oversee client outreach, enrollments, and the creation of Individual Care Plans
  • Assess if clients' needs can be managed by an ECM Lead Care Manager; serve as an escalation point when necessary
  • Drive the team to meet key performance indicators (KPIs) effectively
  • Collaborate with the Director(s) of Enhanced Care Management to implement changes and process improvements
  • Provide clinical oversight approximately 30% of the time, including supervision of clinical staff, clinical sign-offs, oversight of care plans, leading IDT meetings, and serving as a clinical consultant for complex clinical cases
  • Provide direct care management as needed to support organizational goals
  • Communicate directly with ECM staff and occasionally with patients via telephone, telehealth, or during Interdisciplinary Care Team meetings to discuss medication management/adherence
  • Ensure appropriate follow-up with Primary Care Provider, Behavioral Health Provider, and other specialist(s)
  • Guide any crises encountered by ECM staff
  • Collaborate with the client’s broader care team to provide client advocacy
  • Monitor team progress and coach staff to succeed and manage performance
