Manage prescription-related tasks, including refill requests and coordination with providers and pharmacies Handle patient check-in and check-out, ensuring accurate documentation and smooth patient flow Process and track referrals to specialists, including follow-ups and required documentation Create and update patient action plans as directed by providers Maintain accurate patient records and ensure compliance with healthcare documentation standards Communicate professionally with patients, providers, and internal team members via phone and video calls
