Requirements:
Bachelor’s Degree Required in nursing, social work, or other health and human services disciplines. At least one year of clinical care management experience. Some experience directly working with people with cancer or advanced illness. Experience as a Certified Case Manager (CCM), Community Health Worker (CHW) or Peer Support Specialist (PSS), or Accredited Case Manager (ACM) preferred.
Responsibilities:
Engage in initial clinical member biopsychosocial intake including a safety screen and administering BH assessments. Engage in follow-up visits and asynchronous interactions, including brief interventions. Act as a care coordinator and liaison between referring specialists and the practice. Lead weekly interdisciplinary team meetings. Communicate closely with the Cerula Care behavioral health care team. Document member progress in a HIPAA-compliant electronic medical record system. Identify patients not improving and report concerns to medical providers and the Consulting Psychiatrist. Facilitate referrals for clinically indicated services outside of Cerula Care.
