  • Bachelor’s Degree required in nursing, social work, or other health and human services disciplines from an accredited college or university.
  • Experience as a Certified Case Manager (CCM), Community Health Worker (CHW) or Peer Support Specialist (PSS), or Accredited Case Manager (ACM) preferred.
  • Qualified applicants must have at least one year of clinical care management experience, particularly with people with cancer or advanced illness.

  • Engage in initial clinical member biopsychosocial intake, including safety screens and BH assessments.
  • Conduct follow-up visits and interactions, providing brief interventions.
  • Serve as key coordinator between referring specialists and Cerula Care team.
  • Lead weekly interdisciplinary meetings to discuss high-risk members and document new psychiatric recommendations.
  • Maintain ongoing communication with the behavioral health care team.
  • Document member progress in a HIPAA-compliant system.
  • Identify patients needing more intensive care and report to the medical provider.
  • Facilitate referrals for indicated services under supervision.
