Requirements:
Qualified Mental Health Professional (QMHP) holding one of the following clinical licenses, in Texas, Indiana, and/or Tennessee: Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Psychologist (PhD or PsyD), or equivalent. Experience leading, building and implementing programs and delivering outcomes. Strong experience leading a team. Experience with Health Coaching or Medical Social Work preferred. Master's degree in behavioral health, social work or health care management from an accredited university required.
Responsibilities:
Lead a team of Care Extenders to implement programs and serve members. Empower members to improve health and wellness through personalized case management. Seek creative solutions to support Care Extenders and program goals. Act as a player/coach, building Care Extender skills and directly supporting members. Create service plans using available information and knowledge of community resources. Guide members in developing skills and strategies for relapse prevention and risk reduction. Educate members about illnesses, abuse, violence prevention, medication, and available resources. Monitor, evaluate, and record member progress. Build accountability communications for upcoming appointments. Interact with members, families, physicians, and other providers. Report critical incidents and quality-of-care issues. Build communication strategies for team engagement with members. Assess members for risk, perform crisis intervention, and ensure member safety. Develop and coordinate community resource guides. Identify member diagnoses and ensure comprehensive service plans. Lead Care Extender team to ensure adequate assessment and progress. Coordinate caseloads and refer new members. Ensure team schedules meet program needs. Participate in screening, interviewing, and hiring new team members. Provide orientation and training for new team members, conduct performance reviews, and coach the team. Lead enrollment activities. Ensure the team meets Key Performance Indicators. Perform regular documentation reviews for quality and training. Analyze team performance to identify gaps and create improvement plans. Collect and synthesize data highlighting member outcomes. Lead regular internal case review sessions. Participate in external case management meetings with health plan partners. Perform duties to maintain program-related and administrative data/reports. Partner with the billing team to review claims.
