Handle incoming calls from patients and staff regarding clinical questions Collaborate with physicians, other healthcare providers, and community agencies Analyze clinical data and identify opportunities for improvement Promote a positive and supportive work environment for clinical staff Support and educate new and existing staff Provide oversight and care coordination for designated caseload of patients After hours/on call responsibilities to support the team Reviewing and signing Start of Care Orders, recertification orders, resumption of care orders, discharge orders and external orders
