Requirements:
Preferably 2 years of experience working in a healthcare or community-based setting. Ideally, 1 year of experience (professional or lived) in understanding the needs and challenges faced by family caregivers and older adults. Demonstrate a deep understanding of cultural humility and ability to communicate effectively with diverse individuals. Experience working with Electronic Health Records (EHRs) is a plus. Ability to handle interactions with caregivers and patients involving heightened emotions. Strong general computer skills including navigating systems and setting up work equipment. Attention to detail and effective time management and multi-tasking skills. Familiarity with healthcare industry, mental health, aging and disability services, or Alzheimer’s disease. Proficiency in another language, other than English, is a plus.
Responsibilities:
Provide virtual care via telehealth to patients/families in home settings. Act as the primary point of contact for caregivers, families, support networks, and individuals with chronic conditions. Conduct biopsychosocial assessments to identify needs, goals, and concerns. Develop personalized care plans aligned with client values. Provide disease and stage-specific education. Refer and connect individuals to community-based services. Communicate with primary care physicians and other healthcare specialists. Provide caregiver education on burnout, self-care, and resilience. Track and maintain contact frequency. Participate in transdisciplinary meetings. Provide accurate and efficient documentation. Follow company policies, procedures, and clinical protocols.
