Requirements:
Preferably 2 years of experience in healthcare or community-based settings. Ideally, 1 year of experience in understanding the needs of family caregivers and older adults. Demonstrate deep understanding of cultural humility and ability to interact with diverse individuals. Experience working with Electronic Health Records (EHRs) is a plus. Ability to handle emotionally charged interactions. Strong general computer skills. Attention to detail and effective time management/multi-tasking skills. Familiarity with healthcare industry, mental health, aging/disability services, or Alzheimer's disease.
Responsibilities:
Act as the primary point of contact for caregivers, families, and patients. Conduct biopsychosocial assessments and develop personalized care plans. Provide disease and stage-specific education and connect clients to community resources. Communicate with primary care physicians and other healthcare specialists. Provide caregiver education on burnout, self-care, and resilience. Track and maintain contact frequency. Participate in transdisciplinary team meetings. Maintain accurate documentation and adhere to company policies.
