Quality Outreach Specialist

Medical Assistant, Licensed Practical Nurse, Certified Nursing Assistant, Emergency Medical Technician, or Pharmacy Technician license or certification required Bachelor’s/ Undergraduate degree preferred 3+ years of experience in a role closing Healthcare Effectiveness Data and Information Set (HEDIS) measures and/or Star Rating gaps Prior quality measure chart review experience Experience complying with Health Insurance Portability and Accountability Act (HIPAA) Inpatient or outpatient clinical experience with strong electronic medical records (EMR) experience preferred Ability to manage deadlines and handle multiple tasks simultaneously Detail orientated, organized and self-motivated Ability to handle sensitive and/or confidential material and information appropriately Strong verbal and written communication skills Ability to work with large files and data sets Proficient in Microsoft Office Suite (PowerPoint, Excel, Word, Outlook)

Patient outreach calls to address medication non-adherence barriers Connect patients with care and services to close gaps in care Follow up with patients' care teams on conversation findings Review medical records to find evidence of gap closure Submit gap in care closure details to payors and partners Provide expertise on educational tools and learning sessions Identify learning opportunities for practices and providers Support development of initiatives for patient care and gap closure Support creation and review of educational materials for providers
