Palliative Care Physician

Board-certified physician in a relevant specialty with subspecialty training or equivalent experience in Palliative Care. At least two years of post-training clinical experience, primarily in palliative care, complex primary care, or oncology. Unrestricted medical license and willingness to obtain additional state licenses as needed. Demonstrated excellence in virtual or telehealth care delivery, communication, and documentation. Skilled in the care of complex patients, shared decision-making, advance care planning, and interdisciplinary collaboration. Comfortable using technology in daily practice (video visits, EHRs, Google Suite, Slack, AI-enabled tools).

Conduct virtual specialty palliative care visits for symptom management, advance care planning, and psychosocial support. Provide virtual urgent or same-day clinical visits. Partner with an interdisciplinary team to create and monitor individualized care plans. Lead or participate in interdisciplinary case conferences. Collaborate with Nurse Practitioners to ensure coordinated care. Support program development and quality improvement. Communicate and coordinate with external oncology and palliative care providers.
