PT Clinical Navigator (Shift: 8:30AM - 5:30PM EST / 7:30AM - 4:30PM CST)

Doctor of Physical Therapy, Doctor of Physiotherapy (DPTs), or Master of Physical Therapy (MPTs) required Current, unrestricted PT license in Georgia (Compact License strongly preferred) Willingness to obtain additional licensure in new states Minimum 2 years of relevant experience in physical therapy or MSK-related clinical roles Strong written and verbal communication skills Experience using data to optimize clinical workflows Demonstrated ability to navigate fast-paced, evolving environments with autonomy and accountability High comfort level with technology (Google Suite, CRMs, dual-monitor workflows, real-time communication platforms)

Conduct clinical intake assessments Build trust-based relationships with patients Provide education and guidance on treatment options Monitor patient progress and provide motivational coaching Identify and address barriers to care Serve as a liaison between patients and providers Deliver clinical updates Escalate complex cases Partner with care team members Provide peer support Participate in team meetings Accurately document patient interactions Adhere to established protocols for call times and assessments
