Requirements
- Certified Medical Assistant (CMA).
- Bilingual fluency in Spanish and English.
- 1–3 years of healthcare experience.
- Ability to work 8:00am – 5:00pm Central Time.
- Intermediate proficiency with Microsoft Office Suite (Outlook, Teams).
- Experience managing structured, metric-driven workflows.
- Comfortable using EHR systems and patient management platforms simultaneously.
- Home internet with minimum 25 Mbps download / 10 Mbps upload speed.
- Dedicated private workspace for HIPAA/PHI compliance.
- Chronic disease or nephrology experience (preferred).
- Motivational interviewing skills (preferred).
Responsibilities
- Support CCM patient enrollment, onboarding, and eligibility verification.
- Conduct high-volume text and telephonic outreach to enrolled patients weekly.
- Reinforce provider-directed care plans and identify barriers to medication adherence.
- Document patient interactions accurately within CCM and EHR systems.
- Track engagement time to ensure compliance with Medicare CCM billing requirements.
- Facilitate patient access to providers and coordinate care transitions.
- Escalate patient symptom concerns or medication issues to clinical staff.
