Requirements
- Master’s Degree in Social Work, Counseling, or a related field.
- Current California license as an independent clinician (LCSW, LPCC, or equivalent).
- 1-5 years of professional experience as a licensed clinician.
- Proficiency in English and Spanish for medical interpretation and documentation.
- High-speed internet connection supporting video conferencing.
- Proficiency in evidence-based treatments such as CBT, Motivational Interviewing, and Problem-Solving Treatment.
- Experience with behavioral health screening tools like PHQ9, GAD7, and C-SSRS.
- Ability to learn and use new technology platforms including EHR systems.
Responsibilities
- Provide evidence-based behavioral health interventions via the Collaborative Care Model.
- Coordinate behavioral health care with patients' primary care providers.
- Screen patients for behavioral health and substance use disorders using standardized tools.
- Monitor patient symptoms, treatment side effects, and treatment effectiveness.
- Support psychotropic medication management by tracking adherence and effectiveness.
- Document clinical encounters and progress in Salesforce and electronic health records.
- Identify social determinant barriers and coordinate with Care Navigation Partners.
