Requirements
- Master’s degree in Social Work, Counseling, or related field from an accredited program.
- Current, active independent license to practice in the state of California (LCSW, LPCC, or equivalent).
- Minimum 1-5 years of experience as a licensed professional.
- Proficiency in medical interpretation and documentation in English and Russian meeting industry standards.
- Secure, high-speed internet connection capable of supporting video conferencing and cloud-based systems.
- Proficiency in evidence-based treatment approaches (Problem-Solving Treatment, Motivational Interviewing, CBT, Behavioral Activation).
- Expertise in handling higher-acuity patient situations.
- Technical proficiency in EHRs and ability to learn new software platforms.
- Knowledge of behavioral health screening tools (PHQ9, GAD7, CSSRS).
Responsibilities
- Provide evidence-based behavioral health interventions for pediatric and adult populations.
- Support and coordinate care with primary care providers and other service teams.
- Screen patients for behavioral health and substance use disorders using tools like PHQ9 and GAD7.
- Monitor patient symptom changes, treatment adherence, and medication side effects.
- Track clinical outcomes and patient follow-up using a registry.
- Document patient progress and recommendations in Salesforce and EHR systems.
- Collaborate with psychiatric consultants and clinical supervisors through scheduled consultations.
- Identify social determinant barriers and coordinate with Care Navigation partners.
