Requirements
- Master’s Degree in Social Work, Counseling, or a related field.
- Current, independent license to practice in California (LCSW, LPCC, or equivalent).
- 1-5 years of professional experience as a licensed clinician.
- Proficiency in medical interpretation and documentation in English and Mandarin.
- Secure, high-speed internet connection for video conferencing.
- Proficiency in evidence-based treatments (CBT, Motivational Interviewing, Behavioral Activation).
- Experience with behavioral health screening tools (PHQ9, GAD7, CSSRS).
- Technical proficiency with EHR systems and cloud-based platforms.
Responsibilities
- Provide evidence-based interventions via the Collaborative Care Model to pediatric and adult populations.
- Screen patients for behavioral health and substance use disorders using PHQ9, GAD7, and CSSRS.
- Coordinate care with primary care providers and psychiatric consultants.
- Track patient outcomes and document encounters in Salesforce and the electronic health record.
- Monitor treatment effectiveness, medication side effects, and adherence.
- Develop and implement relapse prevention plans with patients.
- Identify and address social determinant barriers alongside Care Navigation Partners.
