Individuals who are proficient in medical interpretation and documentation in multiple languages that meet industry standards are eligible for a 5% adjustment in their base salary.

Requirements
- Master’s Degree in Social Work, Counseling, or related field.
- Current California licensure as an independent clinician (LCSW, LPCC, or equivalent).
- 1-5 years of experience as a licensed professional.
- Proficiency in medical interpretation and documentation in both English and Farsi.
- Secure, high-speed internet connection capable of supporting video conferencing.
- Proficiency in evidence-based treatment approaches (Problem-Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, Behavioral Activation).
- Technical proficiency with EHR systems and ability to learn new technology quickly.
- Knowledge of behavioral health screening tools (PHQ9, GAD7, CSSRS).
Responsibilities
- Provide care to patients based on needs of assigned Care Pod and customize treatment plans.
- Support and coordinate behavioral health care with primary care providers and other service providers.
- Screen patients for behavioral health and substance use disorders using standardized assessment tools.
- Monitor patients for clinical symptoms and treatment side effects or complications.
- Support psychotropic medication management and track treatment adherence and effectiveness.
- Track patient follow-up and clinical outcomes using a clinical registry.
- Plan mutual self-management goals and interventions with patients.
- Document patient progress and recommendations in Salesforce and the electronic health record.
- Identify social determinant barriers and coordinate with the Care Navigation Partner.
