- Active medical license (multi-state licensure a plus)
- Missouri license (residence not required)
- 1+ year of physician experience in family medicine or internal medicine
- Experience in preventive care, senior health, or chronic condition management (plus)
- Experience practicing via telehealth (plus)
- Proficiency with EHR systems (plus)
- Experience providing preventive virtual care (plus)
Responsibilities
- Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
- Create and manage personalized care plans for ongoing conditions
- Collaborate with our integrated care team (nurse case managers, NPs, CHWs)
- Ensure thorough documentation in our EHR and compliance with quality measures
- Engage patients in preventive care to reduce avoidable complications
