Requirements
- MD or DO degree.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics.
- 1+ year of physician experience in family medicine or internal medicine.
- Active medical license (multi-state licensure a plus).
- Experience in preventive care, senior health, or chronic condition management preferred.
- Comfortable practicing via telehealth and using EHR systems preferred.
- Experience providing preventive virtual care preferred.
Responsibilities
- Provide routine telehealth visits with Medicare patients, including E/M and Chronic Care Management.
- Create and manage personalized care plans for ongoing conditions.
- Collaborate with integrated care teams including nurse case managers, NPs, and CHWs.
- Ensure thorough documentation in EHR and maintain compliance with quality measures.
- Engage patients in preventive care to reduce avoidable complications.
