Requirements
- Active compact multi-state LPN/LVN license in the state of residence.
- Active California state LVN license.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Ability to work at least three days per week including required Saturdays.
- Ability to work standard business hours (8:00 AM to 6:00 PM PT).
- Remote work readiness including stable high-speed WiFi.
- Strong interpersonal communication skills for phone-based patient interaction.
- Comfort working independently and navigating multiple systems simultaneously.
- Experience with AI-assisted clinical tools is preferred.
- Fluency in Spanish is preferred.
- Experience providing patient education to individuals managing chronic conditions is preferred.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
- Help patients execute personalized care plans through education on behavior modification, nutrition, and self-management strategies.
- Conduct comprehensive assessments of patients' health status, lifestyle behaviors, and readiness to change.
- Educate patients on disease management, medication adherence, and symptom recognition according to clinical protocols.
- Monitor patients' progress and adherence to treatment plans through regular check-ins and remote monitoring tools.
- Document patient interactions and follow-up plans in the Cadence platform and identify barriers to care.
- Collaborate in real time with clinicians and support teams using Google Workspace, Slack, and the Cadence Platform.
