Requirements
- Active compact multi-state LPN/LVN license in the state where you currently reside.
- Active California LVN license.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Ability to provide patient education and support for chronic conditions.
- Demonstrated experience using AI-assisted clinical tools (e.g., Gemini, Open Evidence).
- Proficiency with Google Workspace and Slack.
- Remote work readiness including stable high-speed WiFi.
- Strong interpersonal communication skills for phone-based patient interactions.
- Ability to work independently in a fast-paced environment with evolving workflows.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions.
- Help patients execute personalized care plans focusing on behavior modification, nutrition, and self-management.
- Conduct comprehensive health status and lifestyle assessments.
- Educate patients on disease management, medication adherence, and symptom recognition.
- Monitor progress and treatment plan adherence through regular check-ins and remote monitoring tools.
- Document patient interactions and outcomes in the Cadence platform.
- Collaborate in real time with clinicians and support teams using Google Workspace and Slack.
