Requirements
- Active compact multi-state LPN/LVN license in current state of residence.
- Active California (CA) state LVN license.
- Minimum 5 years of clinical experience as a Licensed Practical Nurse.
- Experience in patient education and support for chronic conditions.
- Remote work readiness with stable high-speed WiFi.
- Proficiency with AI-assisted clinical tools such as Gemini or Open Evidence.
- Ability to navigate and troubleshoot multiple systems simultaneously.
- Strong interpersonal and telephonic communication skills.
- Discipline in clinical documentation and scheduling adherence.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic condition management.
- Help patients execute personalized care plans focusing on behavior modification, nutrition, and self-management.
- Conduct comprehensive health assessments regarding lifestyle, nutrition, and readiness for change.
- Educate patients on disease management, medication adherence, and symptom recognition.
- Monitor patient progress and treatment adherence through regular check-ins and remote monitoring tools.
- Document patient interactions and outcomes in the Cadence platform while identifying care barriers.
- Collaborate with clinicians and support teams using Google Workspace, Slack, and the Cadence Platform.
