Requirements
- Active compact multi-state LPN/LVN license in current state of residence.
- Active California state LVN license.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness with stable high-speed WiFi.
- Experience providing patient education and support for chronic conditions (preferred).
- Fluency in Spanish (preferred).
- Familiarity with AI-assisted clinical tools like Gemini or Open Evidence.
- Strong interpersonal communication skills for building patient trust over the phone.
- Proficiency in documentation and navigating multiple systems simultaneously.
- Ability to work independently in a fast-paced environment.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions.
- Help patients execute personalized care plans through education on behavior modification and self-management.
- Conduct comprehensive assessments of health status, lifestyle behaviors, and readiness to change.
- Educate patients on medication adherence, symptom recognition, and prevention strategies.
- Monitor patient progress, treatment plan adherence, and outcomes via remote monitoring tools.
- Document patient interactions and outcomes while identifying social needs and barriers to care.
- Collaborate in real-time with clinical teams using tools like Google Workspace and Slack.
