Requirements
- Active compact multi-state LPN/LVN license in the state of residence.
- Active California state LVN license.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness with stable high-speed internet.
- Experience in patient education and coaching for chronic condition management.
- Proficiency with AI-assisted clinical tools such as Gemini or Open Evidence.
- Strong interpersonal communication skills for building trust over the phone.
- Ability to work independently in a fast-paced environment with evolving workflows.
- Tech-savvy to navigate and troubleshoot multiple systems simultaneously.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
- Help patients execute personalized care plans through education on behavior modification, nutrition, and self-management strategies.
- Conduct comprehensive assessments of health status, lifestyle behaviors, and readiness to change.
- Educate patients on disease management, medication adherence, and prevention strategies according to clinical protocols.
- Monitor patient progress, treatment adherence, and health outcomes via check-ins and remote monitoring tools.
- Document interactions, outcomes, and follow-up plans in the Cadence platform and identify barriers to care.
- Collaborate with clinicians and patient support teams using Google Workspace, Slack, and the Cadence Platform.
