Requirements
- Active compact multi-state LPN/LVN license in current state of residence.
- Active California state LVN license.
- Minimum 5 years of clinical experience as a Licensed Practical Nurse.
- Experience in patient education, coaching, and care coordination.
- Remote work readiness with stable high-speed internet.
- Proficiency in navigating multiple systems simultaneously while maintaining patient engagement.
- Ability to use AI-assisted clinical tools while applying clinical judgment.
- Experience with chronic condition management (e.g., diabetes, hypertension).
- Strong interpersonal and telephone communication skills.
- Fluency in Spanish is preferred.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for patients managing chronic conditions.
- Help patients execute personalized care plans focusing on behavior modification, nutrition, and self-management.
- Conduct comprehensive health assessments and evaluate patient readiness for lifestyle changes.
- Educate patients on disease management, medication adherence, and symptom recognition.
- Monitor patient progress and treatment adherence using remote monitoring tools.
- Document patient interactions and follow-up plans in the Cadence platform.
- Collaborate with clinicians and patient support teams to escalate concerns and coordinate care.
